Sanofi (SNY) Q3 2026 Results: Operating Income Up 19.7% as Regeneron Payments End

Published 6 min read

Summary

Sanofi grew Q3 2026 business operating income 19.7% at constant exchange rates on 14.3% sales growth, as flat costs and a richer specialty mix offset the end of Regeneron development reimbursement.

Sanofi's Q3 2026 results show operating income growing much faster than sales, even after two first-half supports fell away. In its third-quarter press release dated October 30, 2026, covering the quarter to September 30, Sanofi reported business operating income (BOI) up 19.7% at constant exchange rates (CER) [1]. Second-quarter profit had included the reversal of more than €200 million of Sarclisa provisions [2] and a one-off Dupixent gross-to-net benefit of about €200 million [3]. The open questions were whether third-quarter profit could still grow, and whether Dupixent, new launches and vaccines could hold their own reference points.

The answer was mostly yes. Dupixent sold more than €500 million more than in the second quarter, new launches grew 31.4%, and vaccines fell 6.6%, a smaller decline than the low-to-mid-teens decline the company had indicated earlier [1][4]. Sanofi raised its full-year guidance. Two caveats remain: US pricing again included a one-off benefit that the company did not quantify by product, and US Beyfortus sales fell by almost 40%. The fourth quarter also carries a headwind from the end of Regeneron's development cost reimbursement.

Sanofi is a Paris-based biopharmaceutical company whose products span immunology, rare diseases, vaccines and mature medicines [5]. It sells mainly to wholesalers, pharmacies, hospitals and governments; in 2025 its three largest customers accounted for 18%, 12% and 6% of net sales [6]. Its core product, Dupixent, is partnered with Regeneron: Sanofi books the sales and shares the alliance profit with Regeneron under their agreement. Dupixent made up 36.0% of Sanofi's 2025 net sales [7].

Operating income grew nearly 20% after the Regeneron reimbursement ended

Third-quarter profit grew faster than the 14.3% sales growth, mainly because of a better product mix and flat costs. BOI was €5.430 billion, a 37.6% margin [1]. The business gross margin rose to 81.0%. The company attributed this to a higher share of specialty care and rare disease medicines, plus an element of favourable gross-to-net adjustments (lower US rebate and discount deductions). R&D expenses fell 0.1% at CER, and selling and general expenses fell 0.9% [1].

Higher Regeneron profit sharing and the end of the development reimbursement offset part of that growth. The Regeneron alliance cost Sanofi €2.399 billion in the quarter, versus €1.369 billion a year earlier, or about 42% of the quarter's Dupixent sales. The additional profit share Sanofi receives for development costs fell to €11 million from €293 million a year earlier [1]. The CFO said this reimbursement ended in the second quarter and creates a BOI headwind of about €500 million in the second half [4]. Amvuttra licensing income rose to €296 million, partly offsetting the impact [1].

Profit growth stays in double digits even after removing this quarter's one-off pricing benefit. The CFO estimated underlying group sales growth at about 12% excluding one-offs, roughly 2.3 percentage points below the reported rate [4]. On the prior-year sales base, that gap is about €290 million. Even if all of it had flowed through to operating profit, BOI growth at CER would still be about 13%. Sanofi raised its full-year guidance to low-teens sales growth and mid-teens business EPS growth [1].

Dupixent, new launches and vaccines

Dupixent sales rose above the second-quarter level, but US growth again included gross-to-net adjustments. Third-quarter Dupixent sales were €5.687 billion, up 34.8% at CER, with Europe up 16.1% and the Rest of World up 22.6% [1]. The company said US growth came from demand and an extended benefit from favourable gross-to-net adjustments. The CFO said 85% to 90% of group growth is sustainable and that the same conclusion applies to Dupixent, without giving a one-off amount for Dupixent [1][4].

New launches kept growing from quarter to quarter, though growth slowed as Ayvakit entered the prior-year base. Pharma launch sales were €1.390 billion, up 31.4% at CER, compared with €1.305 billion in the second quarter. Ayvakit grew 14.4% on a pro forma basis [1]. Sanofi expects its two legacy divestments, to Cheplapharm and of Medley, to reduce 2027 sales by about €1.3 billion [4].

Vaccines declined less than guided, but US Beyfortus fell sharply. Vaccine sales were €3.185 billion, down 6.6% at CER. Influenza vaccines fell 18.8%, which the company attributed to a larger share of deliveries falling in the fourth quarter. US Beyfortus sales fell 39.3%, partly because of a lower central order from the US Centers for Disease Control and Prevention [1]. The company expects full-year vaccine sales to decline by a low single-digit percentage [4].

Conclusion

The third quarter improves the picture of how well Sanofi converts sales into profit. Free cash flow for the first nine months was €6.993 billion, up 28.3%, while net debt rose to €12.201 billion [1]. The one-off pricing benefit has not been quantified by product, and the company expects pricing to make a slightly negative contribution in 2027 [4]. The next tests are whether fourth-quarter profit can keep growing despite the reimbursement headwind, whether Dupixent can hold its third-quarter level, and whether US Beyfortus recovers.
