UnitedHealth (UNH) Q3 2026: MCR Falls to 88.4%, Guidance Raised

Summary
UnitedHealth's Q3 2026 medical care ratio fell 1.5 points to 88.4%, insurance profit already tops full-year guidance, and Optum Health keeps shrinking.
UnitedHealth Group (UNH) reported third-quarter 2026 results on October 13, 2026, in an 8-K earnings release, and its insurance margin recovery is running ahead of the full-year path the company set at midyear, while the business keeps shrinking [1]. Of the four questions that framed the quarter, the medical care ratio improvement held up after removing reserve effects; Optum Health's profit came mainly from operations, but its revenue decline widened to 10%; Optum Rx grew operating earnings; and commercial membership kept falling, with no new cost-side data in the release. The company raised its full-year 2026 adjusted earnings outlook to at least $20.50 per share [1]. The strategy of trading scale for margin has paid off in 2026. The next question is whether 2027 pricing, membership and Optum Health's size can stabilize at the new margin level.
UnitedHealth Group is the largest US health insurance and health services company [2]. UnitedHealthcare collects premiums and bears medical costs, and it covered 48.3 million people at the end of the third quarter [1]. Optum provides care delivery, pharmacy benefit management and health technology, and a large share of its customers are the company's own insurance plans [2].
UnitedHealth Q3 2026 medical care ratio beats the full-year guidance path
The third-quarter medical care ratio (MCR, medical costs as a share of premiums) was 88.4%, 1.5 percentage points lower than a year earlier, which management attributed to cost management, benefit changes and pricing discipline [1]. The quarter included $610 million of favorable reserve development, worth about 0.7 percentage points; excluding it, MCR was about 89.1%, still below the 89.9% reported a year earlier [1]. Days claims payable held at 47.0, flat with the second quarter, so there is no sign that profit came from thinner reserves [1].
The year-to-date numbers show the company's midyear full-year guidance now looks conservative. MCR for the first nine months was about 86.3%, while the full-year guidance the company gave in the second quarter was 88.1% plus or minus 25 basis points [1][3]. Assuming fourth-quarter premiums are close to the third quarter's, the fourth-quarter MCR would need to be about 4 percentage points higher than the third quarter's for the full year to land in that range. UnitedHealthcare's operating earnings for the first nine months reached $12.467 billion, already above the company's full-year guidance of more than $12 billion [1][3]. Medicare Advantage membership rose by 40,000 from the end of June, indicating that this year's planned membership reduction is complete [1]. The release did not restate the full-year MCR guidance or the Medicare margin commentary, so both remain to be confirmed.
Optum Health profit comes from operations, but shrinkage is accelerating
Optum Health's third-quarter profit beat the company's earlier seasonality guidance. In the second quarter, management said nearly all of the segment's full-year earnings would be recognized in the first half [4]. Third-quarter reported operating earnings were $735 million, and $588 million after excluding divestiture, restructuring and other items; loss contract reserves fell by a net $95 million in the quarter, so most of the profit came from operations [1]. Reported operating earnings for the first nine months were $3.066 billion, already above the full-year guidance of more than $2.275 billion [1][3].
That recovery came with faster shrinkage. Third-quarter revenue fell 10% year over year, which the company attributed to serving fewer value-based care patients in markets where it lacked sufficient scale, and consumers served fell from 93 million to 92 million [1]. The release did not separate how much of the decline reflects divestitures or other scope changes in the prior-year base.
Optum Rx grows earnings while commercial membership keeps falling
Optum Rx raised operating earnings even as script volume declined. Third-quarter operating earnings were $1.718 billion, up 10.9% year over year, while adjusted scripts fell 5.1%, a smaller decline than in the second quarter; the company cited membership declines at both UnitedHealthcare and other customers [1].
Commercial membership losses have not stopped. Employer & Individual lost 165,000 people in the third quarter due to employer attrition, and fee-based membership declined sequentially for the first time this year; the business line's revenue rose 1.7% year over year, but domestic revenue grew only 0.9% [1]. The release did not update the out-of-network arbitration cost or the commercial cost trend.
Conclusion
The quarter confirms that UnitedHealth's profit conversion has improved materially, while growth remains secondary to margin. Consolidated operating earnings were $6.563 billion, compared with $4.315 billion a year earlier, and operating cash flow for the first nine months was $25.087 billion, compared with $18.589 billion [1]. Revenue fell 0.4% year over year, and medical membership was 1.46 million lower than at the start of the year [1]. The next tests are whether fourth-quarter MCR keeps improving year over year, whether 2027 pricing and membership plans preserve the margin, and when Optum Health's shrinkage bottoms out.
