Summary

Centene raised 2026 adjusted EPS guidance to above $5.20 as Medicare beat, while Commercial HBR jumped to 89.0% from 79.2% on unquantified IDR reserve additions.

Centene (CNC) Q3 2026 earnings, reported on October 27, 2026 through its earnings release, 10-Q and conference call, raised the company's full-year profit outlook even as quarterly profit in its Marketplace business narrowed sharply [1][2][4]. Centene raised its full-year 2026 adjusted diluted EPS guidance from greater than $4.80 to greater than $5.20 [1]. Of the three business lines in focus, Medicaid's match between rates and rising member acuity ran on plan, Medicare beat expectations in its peak drug-cost quarter, and Marketplace moved its full-year margin outlook higher even though its quarterly cost ratio jumped [2]. Taken together, the profit recovery is still progressing, but the Marketplace recovery was largely locked in during the first half, leaving little room in the second half.

Centene is the largest US Medicaid and ACA Marketplace health insurer and the largest standalone Medicare prescription drug plan (PDP) provider [6]. Governments and individuals pay monthly premiums, and the company bears its members' medical and drug costs. The share of premiums spent on medical costs is the health benefits ratio (HBR); a higher HBR means thinner profit.

Marketplace cost ratio jumped in Q3 while the full-year margin outlook moved up

The Commercial segment's Q3 HBR rose to 89.0%, far above 79.2% in the second quarter [1][3]. Excluding a roughly $180 million settlement of prior-year risk adjustment, the Q2 figure was about 80.8%, and the earlier expectation was only a seasonal second-half rise of 3 to 4 percentage points. Segment gross margin was $958 million, compared with $1,947 million in Q2 and $1,116 million a year earlier [3][4]. The year-over-year decline mainly reflects Marketplace membership falling from 5,828,100 to 3,403,300 [1][4].

Management attributed the Q3 jump to three factors but did not disclose amounts [2]. The company strengthened reserves for independent dispute resolution (IDR), the process through which providers take billing disputes to third-party arbitration; it also adjusted program-integrity reviews and absorbed normal seasonality from bronze-tier members [2]. Management said the reserve catch-up should not recur in Q4, but how much of the jump it explains cannot be verified. The segment also includes about $4 billion of commercial group business [2].

The full-year picture improved [2]. Management moved its full-year Marketplace pre-tax margin expectation to the top of its 4.5% to 5% range and said the latest risk adjustment data was in line with expectations [2]. At September 30, the risk adjustment receivable was $2,334 million and the payable was $1,353 million [5]. The nine-month Commercial HBR was 80.9%, compared with 85.3% a year earlier, while the Q3 figure was already close to the prior-year quarter's 89.9% [1]. The full-year recovery view still holds, but the profit came mainly from the first half [2].

Medicare beat expectations and Medicaid stayed on plan

Medicare was the leading source of the guidance increase [2]. The Medicare HBR was 93.8% in Q3, compared with 94.3% a year earlier, and segment gross margin was $699 million, up 30% [1][4]. As of September, the company had recorded no premium deficiency reserve for the 2026 Medicare Advantage (MA) contract year [5]. Management expects MA to break even this year, one year ahead of plan, and now expects PDP full-year pre-tax margin in the mid-to-high 3% range; it expects the 2027 PDP margin to start in a 2% to 3% range [2].

Medicaid stayed on its full-year path [2]. The Medicaid HBR was 93.6%, 30 basis points better than in Q2, and the full-year expectations of about 93.5% HBR and a roughly 5% composite rate increase were unchanged [2]. Membership was down 5% from year-end 2025, and the company held its expectation of an 8% to 9% full-year decline [1][2]. Part of the rate cushion set aside for the second half was used in Q3, and the membership decline is expected to be larger in Q4 [2].

Conclusion

The quarter strengthened the case for Centene's earnings recovery but reduced visibility into second-half Marketplace profit [1][2]. Premium and service revenues grew only 0.5% year over year, and the adjusted SG&A expense ratio fell from 7.0% to 6.5% [1]. Operating cash flow was an outflow of $3.7 billion in Q3, mainly because of Medicaid pass-through payments, and nine-month operating cash flow was $4.3 billion [1][2]. Management set an adjusted EPS jumping-off point of about $4.70 for 2027 and expects growth from there [2]. The next tests are whether Marketplace needs another reserve catch-up in Q4, whether the risk adjustment receivable is revised down, whether January 2027 Medicaid rates cover acuity shifts from work requirements, and whether MA can expand its margin after the PDP margin resets lower.
