Summary

IQVIA Q3 2026 revenue rose 8.0% to $4.43 billion as clinical bookings reached $3.2 billion and next-12-month backlog revenue grew 10%; full-year guidance was raised.

IQVIA's Q3 2026 earnings showed clinical demand accelerating: revenue, Adjusted EBITDA and Adjusted Diluted EPS for the quarter ended September 30 all came in above the high end of company guidance, according to the 8-K earnings release, 10-Q and earnings call published on October 27, 2026 [1]. Going into the quarter, three questions mattered: whether the clinical backlog would convert to service revenue at about the 7.5% pace of its next-12-month revenue, whether launch services in Commercial Solutions would keep offsetting slow growth in information products, and whether productivity would absorb pass-through costs and turn profit into cash [2][3]. Backlog conversion and margin expansion were both confirmed this quarter. Commercial Solutions growth slowed, and quarterly cash flow was weaker than a year earlier. Overall, growth and profit conversion strengthened, while cash generation was steady rather than strong.

IQVIA is one of the world's largest providers of outsourced clinical research and pharmaceutical data, and its clients include almost all large drugmakers and biotech companies [4]. About 60% of revenue comes from Research & Development Solutions (R&DS), which runs clinical trials for sponsors. About 40% comes from Commercial Solutions, which sells data subscriptions, analytics and consulting, and patient and sales services for newly launched drugs [5].

Clinical bookings and near-term backlog accelerated together

R&DS revenue growth and bookings both ran ahead of second-quarter levels. Segment revenue was $2,639 million in Q3 2026, up 8.7% year over year, with organic growth of almost 7%. Excluding reimbursed expenses, R&DS revenue grew 6.8%, compared with 6.7% in Q2 [1][6][2]. Reimbursed (pass-through) expenses are payments to investigators and trial sites; they are recorded in both revenue and cost and carry almost no profit.

Bookings support the case that faster revenue growth can continue. Net new bookings were $3.2 billion, up 15% year over year, for a book-to-bill ratio of 1.2x. Last-twelve-month net new bookings reached $11.7 billion, up 14%, and have risen for five consecutive quarters [1]. Management said full-service bookings were very strong for a second straight quarter, win rates rose two to three points, and cancellations stayed in the historical range [6].

The backlog cleanup did not reduce near-term revenue coverage. IQVIA completed its review of inactive trials and removed about $2.1 billion from backlog, using a test of trials inactive for more than 18 months with no expected near-term restart. Contracted backlog was $32.6 billion on September 30 [1][6]. The company said the adjustment does not affect historical results, full-year guidance or next-twelve-month revenue. It expects about $9.4 billion of backlog to convert to revenue over the next twelve months, up 10% year over year, versus $9.23 billion and 7.5% growth at the end of June [1][2].

The limit of this progress shows up in profit. Full-service work brings more pass-through costs, and the R&DS segment margin was 21.6%, roughly flat with 21.5% a year earlier [5].

Margins expanded and full-year guidance rose

IQVIA expanded margins even as pass-through costs increased. Q3 Adjusted EBITDA was $1,032 million, up 8.7% and above guidance of $1.0 billion to $1.02 billion. The Adjusted EBITDA margin was 23.3%, up 20 basis points year over year [1][3]. Management said pass-throughs cut gross margin by about 70 basis points and currency by about 10 basis points, and that roughly 80 basis points of operational improvement from cost control and AI automation of its own processes more than offset them [6]. That split is a verbal management figure and cannot be checked independently against the financial statements.

Most of the improvement came from Commercial Solutions. The segment margin was 22.9%, 1.6 percentage points higher than a year earlier [5]. IQVIA raised full-year Adjusted EBITDA guidance from $4.0 billion to $4.05 billion to a new range of $4,025 million to $4,055 million, with the margin kept at about 23.2% [1][3].

Cash conversion improved from Q2 but was weaker than last year. Free cash flow was $667 million in Q3, above $360 million in Q2 and below $772 million a year earlier. Nine-month free cash flow was $1,518 million, up 1.9% [1][7]. The company used cash to cut debt, and the net leverage ratio fell from 3.59x to 3.45x. After issuing 6.375% senior notes in September, management guided to fourth-quarter net interest expense of about $210 million [1][5][6].

Conclusion

The quarter strengthens the view that clinical outsourcing demand is recovering: bookings, near-term backlog and revenue accelerated together, and full-year guidance went up. Profit conversion also improved, but pass-through costs absorbed the efficiency gains in the clinical business, so margin expansion relied mainly on Commercial Solutions. Commercial Solutions organic revenue growth was 4%, below about 5% in Q2, so faster launch-services growth is not yet confirmed [6][8]. Management cautioned that fourth-quarter comparisons are difficult and that currency is a headwind of about 70 basis points [6]. The next tests are whether R&DS can hold its conversion pace against a high prior-year base, whether Commercial Solutions organic growth returns to 5%, and whether full-year cash flow keeps growing under higher interest costs.
